What to include
Explain what happened, when it happened, how it affected you and the outcome you would like. Include a way for the team to contact you. Ask about a suitable way to share confidential documents or sensitive information.
Independent guidance
The Care Quality Commission explains how to complain about adult social care and how to share feedback with the regulator. CQC uses feedback to inform its work but does not take forward individual complaints on your behalf.
If you remain unhappy after complaining to a care provider or council, the Local Government and Social Care Ombudsman may be able to consider the complaint. Follow the guidance for the appropriate next steps.
